How do I replace a Lost or Stolen California BSIS Unarmed Security Guard Card?

Important Information about Lost, Destroyed or Stolen License, Registration, Certificate, or Permit
Security Guards and Proprietary Private Security Officers with Lost Registration Cards must notify your employer and apply to the Bureau for a replacement card within 72-hours of your registration card being lost, destroyed or stolen.
Requesting a Replacement License, Registration, Certificate or Permit
Guards who have lost or had a guard card stolen must submit a card replacement request in one of two ways:

Complete the form (linked below), include a check made out to the Bureau of Security and Investigative Services with the applicable fee, and mail to:

There is a $25 replacement fee.
